Applicants must possess a high school diploma with a minimum grade point average of 2.6 OR a GED Score between 660-800, with no individual test score below 165, upon starting the program and be at least 18 years of age upon starting the clinical rotation. To apply to the School of Phlebotomy Technician you will need to:

• Complete the application.
• Contact a minimum of three references and have them send letters of recommendation to the ¸Û°Äͼ¿â RMH School of Phlebotomy Technician. At least two of the references must be from high school teachers, college professors, or employers.
• Send the School an official copy of your high school transcript and any college courses.
• Submit your personal written statement.

Admission criteria include a personal interview, analysis of transcripts, review of three letters of recommendation, and evaluation of personal written statement. In addition, essential functions are required for admission. If you are accepted into the School, a $100 deposit will be required in addition to tuition.

NOTE: We will prepare you for the lab portion of the ASCP exam or any certification exam. We cannot change the non-lab (experience and/or undergraduate accreditation) requirement for any certification exam. We cannot guarantee that you will be able to sit for any exam.

Students are admitted twice a year for classes August to December with graduation in December and classes January to June with graduation in June.

Tuition

Tuition for the program is $2,000. Tuition for the program includes the lecture and clinical portions, training in basic life support, and liability insurance for the student while in the program. An additional $150 graduation fee will be billed in the final two months of the program. The tuition is refundable according to the Refund Policy.

Refund Policy

If a student withdraws from the program, a refund may be requested. Notice of withdrawal should be submitted in writing to the Program Director of the School of Phlebotomy Technician. (This refund policy applies to the $2,000 tuition).

The refund policy is as follows:

1. A student who enters the school but withdraws or is terminated during the first quartile (25 percent) of the program shall be entitled to a minimum refund amounting to 75 percent of the cost of the program.
2. A student who withdraws or is terminated during the second quartile (more than 25 percent, but less than 50 percent) of the program shall be entitled to a minimum refund amounting to 50 percent of the cost of the program
3. A student who withdraws or is terminated during the third quartile (more than 50 percent, but less than 75 percent) of the program shall be entitled to a minimum refund amounting to 25 percent of the cost of the program.
4. A student who withdraws after completing more than three quartiles (75 percent) of the program shall not be entitled to a refund.

A student applicant may cancel by written notice their enrollment at any time prior to the first class day of the session for which application was made. When cancellation is requested under these circumstances, the school will refund all tuition paid by student, less a deposit fee of $100. A student applicant will be considered a student the first day of class.

Essential Functions

The following essential functions are required for admission to the program:

1. Manual Dexterity: Ability to use hand(s) or prosthetic devices with coordination.
2. Fine Motor: Ability to manipulate small objects with fingertips or adaptive devices.
3. Mobility: Ability to maneuver in the laboratory and around instruments and in patients care settings.
4. Vision: Ability to distinguish red, yellow, and blue colors; distinguish clear from cloudy, and distinguish objects through a microscope.
5. Speech: Ability to verbally communicate understandably in English.
6. Hearing: Ability to adapt with assistive devices (i.e., phone receivers, hearing aid, etc.)
7. Writing: Ability to communicate effectively in the written form in English.
8. Reading: Ability to read, understand and follow directions printed in English.
9. Psychological Stability: Ability to demonstrate the emotional health required for full utilization of the applicant’s intellectual abilities.  Must be able to recognize emergency situations and take the appropriate actions.

Students entering the ¸Û°Äͼ¿â RMH School of Phlebotomy Technician must be able to sign the following statement:
